Fostering a Pet Means Compassionate Temporary Care

Not every animal lover is in a position to adopt a pet. Sometimes permanent, legal ownership is just not possible. For many Stark County Humane Society supporters, fostering a pet is a great option. The animals benefit from compassionate temporary care until their permanent homes are located. You get the short-term companionship of an animal that needs your help and the long-term peace of mind in knowing that you’ve make a real difference.

If "Yes," stop here. Fosters for other groups/organizations are ineligble for SCHS Foster Programs.

Note: Please select only one. Community Foster participants are not eligible for Foster the Fuzzy program, and Foster the Fuzzy participants are not eligible for Community Foster program.
I am willing to foster:
I am at least 21 years of age
I currently rent or own my residence
I have permission from my landlord (renters only)
I give permission for an authorized agent of SCHS to visit my residence.
Will you be moving in the near future?
If the pet/pets I am fostering cause any damage to my residence, including chewing of walls, furniture, etc., I will assume full financial responsibility.
I will take all foster pets to the veterinarian appointments as advised, either by making an appointment at SCHS or with an SCHS authorized veterinarian.
I will contact SCHS after-hours phone number (234) 410-7207 for any emergencies outside normal business hours.
If I choose to utilize any outside veterinary care, I will assume all financial responsibility.
I will attend all scheduled appointments or reschedule in a timely manner.
I understand that if I am 15 (or more) minutes late to a scheduled appointment, I will be required to reschedule at a later date.
I will return all foster pets to SCHS as needed for care.
I understand that vaccines need to be administered on the required dates and failure to attend scheduled appointments may disqualify me from the program.
I currently have other pets in the home.
All of my pets are spayed or neutered.
Is it okay for SCHS to verify?
I certify all of my pets are current on vaccinations.
I certify all of my pets are friendly with other animals.
I certify that all foster animals will be kept isolated from my companion animals.
Furthermore, I agree that SCHS can not guarantee the health or behavior of any animal and will not be held responsible for any illness or injury inflicted upon my personal pets.
I certify that all of my pets are inside companions.
I verify all foster pets will be cared for while I am at work.
I certify that my children are good with animals.
Is anyone in the household allergic?
I certify I have asked any and all questions or concerns regarding fostering and will utilize SCHS as needed to aid me in being a successful foster.
I understand that SCHS reserves the right to discontinue my participation from the foster program at any time.
I certify that if any of my foster animals or companion animals acquire any contagious illness, I will notify SCHS immediately. Failure to do so may lead to program disqualification.

By filling out this form, you are certifying that all of the above information is true. I agree that I will return any pet that I foster to SCHS at the end of the necessary foster time. I will return all foster pets only to SCHS and will not attempt to rehome on my own. Failure to comply will terminate my entitlement to foster for SCHS. I acknowledge my failure to return any foster pets to SCHS as scheduled may result in legal action.
